1. Summary of the Position

The Community Health Officer will play a key role in planning, implementing, and monitoring

community-based health activities under YSYD’s health and protection programs. This position is

primarily field-based and involves regular community centers visits, coordination with health actors, and

active case and patient follow-up. The officer will be responsible for identifying individuals with specific

health needs, facilitating their access to appropriate services, and ensuring continuity of care through

close coordination with healthcare providers and referral mechanisms. The role also includes delivering

health awareness sessions, supporting community mobilization efforts, and documenting field activities

and health trends. Working closely with other program staff and local stakeholders, the Community

Health Officer ensures that services are culturally sensitive, inclusive, and responsive to the needs of

vulnerable populations

Duties and Responsibilities

Technical and Programmatic Responsibilities

· Under the supervision of the Project Manager, provide technical management to project staffs.

· Plan, coordinate, and implement community-level health activities in close collaboration with other

program components and external stakeholders, including local NGOs and community-based

organizations.

· Supervise, train, and provide on-the-job mentoring and capacity building to field staffs to ensure

quality and consistency in their outreach and service delivery.

· Conduct regular field visits to support field staffs, monitor implementation of activities, and provide

technical assistance as needed.

· Oversee the proper use of referral slips by project staffs and ensure follow-up mechanisms are in place

to track referred cases and health outcomes.

· Randomly conduct post-visit follow-ups in communities to assess understanding and retention of

health messages delivered by case workers.

· Compile and analyze weekly reports submitted;prepare and submit consolidated reports to the Project

Manager

· Coordinate with the MEAL team to design data collection tools and compile CHW-related data.

· Regularly review and analyze community health data with relevant technical leads to identify trends

and inform responsive programming.

· Ensure that all CHW-related activities and indicators are achieved on time and within the allocated

budget.

Coordination and Integration

· Work closely with internal departments to ensure integrated, multisectoral implementation of

community health services.

· Establish and maintain effective communication with health service providers, community leaders, and

other stakeholders.

Qualifications and Requirements:

· Must be a citizen of the Republic of Türkiye.

· Bachelor's degree or higher in Medicine, Health Sciences, or other related fields from a recognized

university.

· Minimum of 2 years of professional experience in similar roles.

· Previous experience working with refugees, returnees, or internally displaced populations is an asset.

· Experience in the humanitarian aid or non-governmental sector is an advantage.

· Proficiency in both written and spoken Turkish and English; strong reporting and communication skills

in both languages.

· Knowledge of Persian (Farsi) is an asset.

· Familiarity with case management principles is preferred.

· Strong initiative, motivation, and a commitment to teamwork.

· Eagerness and willingness to learn from others.

· Ability to multitask and handle competing demands under tight deadlines.

· Capacity to establish and maintain effective working relationships in multi-ethnic, multicultural, and

multidisciplinary environments.

· Strong problem-solving and organizational skills; flexible and sensitive to diverse needs.

· Willingness and ability to travel frequently and conduct active field visits.

· Computer and digital literacy, particularly in Microsoft Office programs (Excel, Word, PowerPoint, etc.).
